• A completed and signed copy of Hajj Registration form (click here to get this form)
• A passport that is valid for at least 6 months from the date of departure with a minimum of 4 empty pages.
• A completed and signed Visa Application for Hajj form (click here to get this form)
• 2 Passport size photos
• Vaccination certificate signed and stamped by a doctor:
  • H1N1 Vaccination
  • Adults & children over two years: Meningitis ACYW-135.
  • Children under two years: Two dose of (A) Vaccine with one to two months between each
• 2 Bank Drafts for each pilgrim both written to the order of “Unified Agents Office”. First draft with the amount of $116.00, which is equal to 594 SAR, second draft with the amount of $158.40 which is equal to 435 SAR. (Not Required for Umrah)
• A pre-paid return airway bill of a carrier of your choice must be included for securely returning your processed documents.

Proof of relationship:

• Married Couples: Marriage certificate.
• Children under 18 years: Birth certificate showing the name of the father and the mother
• Married ladies traveling with their relatives: notarized letter from her husband by a notary public, with copy of ID with a signature of her husband. (click here to get this form)

• Single, divorced or widow ladies: notarized letter from her Muhram by a notary public, with copy of ID with a signature of her Muhram. (click here to get this form)

• A letter from an Islamic institution on a formal letterhead to certify conversion to Islam (click here to get this form)

• Must submit colored copy of Permanent Resident card (Green Card) H1 - Visa Holder- a copy of your H1 or Work Permit or Re-Entry Permit.
